CD4 count is still the measurement of Immunity status of HIV PATIENTS,IN MOST PART OF WORLD HIV TREATMENT STILL STARTS ON THEIR STATUS as most start it if it is below 350 or WHO recommends it if it is below 500 but now it has been stated clearly that START TREATMENT IRRSPECTIVE OF CD4 COUNT IMMEDIATELY EVEN IF CD4 VERY HIGH AS SOON AS INFECTION IS DETECTED.
Salim Abdool Karim, M.D., Ph.D., the director of the Centre for the AIDS Programme of Research in South Africa, raised another emerging conundrum in the clinical community: "How does one identify those patients with immune dysfunction,it is by CD4 count only ?"
One's instinct might be to say: "That's what CD4+ cell counts are for." But there is clearly an imperfect correlation between a person's CD4+ count and their immune health, as evidenced by the number of HIV-positive individuals who develop opportunistic infections despite high CD4+ counts. Other mechanisms appear to be at work that a clinician is unable to see using a CD4+ test alone.
In many patient too inspite of starting HIV treatment CD4 count doesnot raise markedly increase slowly although VIRAL LOAD DECREASES OR PATIENT IS CLINICALLY IMPROVED .
